A single resource you can use to take an active role in your health and engage with your Providence Health Plan Powered by Collective Health plan. It provides useful information about your My Collective® account and how to register, about Member Advocates and Care Navigation, and other programs available for Providence Health Plan Powered by Collective Health members.

My Collective® mobile app and online portal
Manage everything from one app.
With the My Collective app, you will find everything you need in one place. Check your benefits, track your claims, see your coverage, and find care all from one easy-to-use app. Need help? A real person is ready to talk or text. It’s support on your terms, with no referrals and no phone trees.
- My Collective is available in English and Spanish.

- Download the Collective Health app from the Apple Store or Google play.
After Open Enrollment, you'll receive a welcome email to register your Collective Health account. Make sure to download the app so you're ready on January 1, 2026.

myProvidence
myProvidence is available for members to access their medical, behavioral health and pharmacy claims for services received before January 1, 2026.
- Medical and behavioral health claims will continue to be available for 6 months following January 1, 2026.
- Pharmacy claims with remain available for 30 days after January 1, 2026.

Your primary care provider (PCP) is your main doctor who helps you stay healthy. Regular visits can catch health problems early when they're easier to treat. They provide check-ups, shots, and screenings.

Behavioral health resources
Galileo - 24/7 virtual behavioral health support
Galileo’s doctors and therapists offer evidence-based support through health coaching and a self-guided Cognitive Behavioral Therapy (CBT) skills program. These tools are designed to help manage stress, anxiety, depression, sleep difficulties, and smoking cessation. If in-person care is needed, Galileo’s Care Navigation team is available to provide referrals and ensure timely access to the right support.
Behavioral Health Network
Our broad behavioral health network gives you access to in-person and virtual providers that fit your needs. This includes therapists, counselors, psychiatric nurse practitioners, psychologists, and psychiatrists. They provide ongoing care and support for many behavioral health challenges. You can get the care you need anytime, from anywhere.
Log in to your My Collective® account and use the Get Care provider search to find a provider.
Behavioral Health Concierge
You and your family members can access virtual and confidential appointments at no cost. This service is a convenient way to seek help from a licensed behavioral health provider. Behavioral Health Concierge is available to members and their eligible dependents in Oregon, Washington, Idaho, Montana, California, and Texas.

Pharmacy resources
Plan formulary
A list of FDA-approved medications, including brand-name and generic options, as well as medications that need prior authorization, follow a step therapy program, or have prescription refill limits. This comprehensive prescription drug formulary designed to promote safe, effective and affordable drug therapy.
Ways to search your plan formulary:
- Use the alphabetical list to search by the first letter of your medication.
- Search by typing part of the generic (chemical) or brand (trade) names.
- Search by selecting the therapeutic class of the medication you are looking for.
Save on eligible specialty medications with HelpScript
A service that may help you lower your copay cost on certain high-cost medications.
HelpScript may reduce your copay to $0 on select specialty medications. Eligible members are enrolled automatically. A specialized patient navigator will assist you every step of the way.
Service Features and Eligibility:
The HelpScript service may help lower your out-of-pocket costs to $0 for eligible specialty medications. In some cases, you may have a small fee which you will be reimbursed for via check within 45 days.
- There is no cost to members for participating in the HelpScript service.
- The medications currently eligible for the HelpScript service are listed below. Medications eligible for the service are subject to change.
- Because this program provides a $0 cost not subject to deductible, the tax laws regarding a health savings account makes the service unavailable to those enrolled in a Health Savings Account (HSA) medical plan.

Our preferred retail pharmacy network allows you to save time and money when getting prescription drugs. With this network, you have access to more than 34,000 participating pharmacies, including retail, preferred retail, mail-order, and specialty locations nationwide.

Providence laboratory services
Your laboratory services are performed and processed at a facility based on your location. If you receive laboratory services in the following counties, ask that your lab work be sent to the nearest Providence hospital or visit your nearest Labcorp:
- Clackamas
- Clatsop
- Hood River
- Jackson
- Multnomah
- Yamhill

Kaia Health provides convenient self-care through a mobile app for any type of pain—sudden, occasional, or chronic. You'll get pain relief, unlimited access to health coaching, personalized physical exercises, and relaxation techniques, all at no cost to you.
Basecamp Prevention and Wellness classes and programs are available both in-person and online, locally and globally, providing you with the tools and support you need to start and continue your personal health journey. We're here to support you every step of the way.

In-person classes are available at the Providence St. Vincent Medical Center (PSVMC) campus. Classes take place in The Well fitness studio and conference rooms in the Mother Joseph Plaza Building.

Quitting tobacco of any sort takes a commitment. And keeping your commitment can be challenging if you're trying to do it on your own You can get the help you need with one of our award-winning support groups and telephone counseling programs:
- Tobacco quit line
A free telephone service to help you develop a personalized quit plan and find counseling and resources in your area. Call 1-800-QUIT-NOW or 877-2NO-FUME (Spanish).
- Free & Clear
A telephone-based program designed for smokers who want a structured program, but are unable to attend regular classes. Call 800-292-2336.

- Cost Estimator Tool
You can estimate the cost for health care services using your My Collective® cost estimator tool. Log in to your My Collective account to get started.
- Click "Get Care" at the top of the screen.
- Select the Cost Estimator search type
- Enter the medical procedure you're searching. You can either use a keyword (like "kidney" or "bariatric") or a CPT code (a type of billing procedure code).
- Enter the location near where you want to get care. You can filter results by distance from this location on the next screen.
- Click the "Search" button. You can then browse through the result on the map or the list.
