How to enroll

There are several ways to enroll in Providence Medicare Advantage Plans. Choose the one most convenient for you.

• Enroll online. You can shop for plans and enroll online with our secure enrollment form.
• Enroll by phone by contacting the Providence Medicare Advantage Plans Sales team at 1-888-226-7338 (TTY: 711). Service is available between 8 a.m. to 8 p.m. (Pacific Time), seven days a week (Oct. 1–Dec. 7) or Monday through Friday (Dec. 8–Sept. 30).
• Enroll by mail by completing a paper enrollment form and mailing to:

  You can also fax it to 503-574-8653
• Enroll with a Providence Community Representative at a virtual meeting*, schedule a one-on-one meeting, or enroll with an agent.

After enrolling, you will receive a notice in the mail acknowledging receipt of your enrollment request.

Individuals must have both Part A and Part B to enroll.
