How to enroll

Important: Providence Medicare Advantage Plans will not be available in 2027. If you enroll in a 2026 Providence Medicare Advantage plan, coverage will end 12/31/26.

If you still want to enroll in a 2026 Providence Medicare Advantage plan, you can enroll using a paper enrollment form. See instructions below.

• Enroll by mail by completing a paper enrollment form and mailing to:

  You can also fax it to 503-574-8653
  

After your enrollment is accepted, you will receive a notice in the mail acknowledging receipt of your enrollment request. If you have any questions, contact the Providence Medicare Advantage Plans Sales team at 1-888-226-7338 (TTY: 711). Service is available between 8 a.m. to 5 p.m. (Pacific Time), Monday - Friday.

Individuals must have both Part A and Part B to enroll.
