Important Notice: Phia Group Data Breach
What happened
On December 4, 2025, Phia Group, a former third-party vendor that provided claims-related subrogation services to Providence Health Plan, Providence Health Assurance and certain employer-sponsored plans, notified Providence of a security incident.
Phia Group identified suspicious activity in its computer network in July 2024. Following a detailed investigation, Phia Group confirmed in December 2025 that some Providence Health Plan and Providence Health Assurance member information may have been involved and promptly notified Providence so that appropriate notifications and support could be provided.
What you can do
If you are affected by this issue, members will receive a notification letter from Phia Group that may include enrollment instructions for complimentary credit monitoring and identity theft recovery services, as well as information about steps individuals can take to help protect themselves against identity theft and fraud.

Further, it is always a good idea to review credit reports for accuracy. Under U.S. law, individuals are entitled to one free credit report annually from each of the three major credit reporting bureaus: Equifax, Experian, and TransUnion.
