Providence Care Management helps you better understand your health so you can take an active role in improving it. Whether you need help with understanding a new diagnosis or assistance navigating options for a diagnosis that has been affecting your life for a long time, we've got you covered.

Our team of Registered Nurses, Social Workers, Clinical Support Coordinators, Behavioral Health Navigators and Technicians provide personalized support with health conditions including, but not limited to:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Heart failure
  • Coronary Artery Disease
  • Diabetes
  • Cancer
  • Parkinson's disease, Multiple Sclerosis, Hemophilia, and Scleroderma
  • Complex health issues requiring hospitalization, rehabilitation, or extensive outpatient therapies
  • Mental health and substance abuse

Our care managers are here to help you with:

  • Health care navigation assistance with pre-service authorizations, finding a provider, medication options, caregiver support, dental care, behavioral health care, end-of-life planning, and much more
  • Personalized health education about medical concerns including new innovations, medication therapy, and symptom management
  • An individualized plan developed with you to help you reach your health goals
  • Ongoing one-on-one support
  • Coordination between your providers
  • Advice on general health and lifestyle choices, including nutrition and exercise, to help reduce risks

Care management services are available to you at no cost. Participation is voluntary and declining/opting-out may be done upon contact with your care manager or contacting us by phone.
