Providence Health Plan earns 4-stars from NCQA

Providence Health Plan’s Commercial and Medicare Advantage plans have earned 4-star ratings from the National Committee for Quality Assurance (NCQA) in its 2025 Health Plan Ratings, placing us in the top 32% of health plans nationwide.


These ratings reflect our commitment to delivering high-quality care and supporting the health and well-being of our members.


What the NCQA ratings mean


NCQA evaluates health plans based on:

  • Quality of care members receive

  • Member satisfaction

  • Health plans’ efforts to improve care and outcomes


Ratings are based on nearly 50 measures, including HEDIS® scores and the Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey. These assessments track patient experience and outcomes across a wide range of services.


Leading in chronic condition care


Providence Health Plan stood out as the only accredited Commercial plan in Oregon to earn a 4.5-star rating in diabetes management – the highest in the state. Our Commercial plans also earned 4-star ratings in:.

  • Heart disease management

  • Cancer treatment

  • Behavioral health care


These results place us among the top-performing health plans in Oregon, alongside peers such as Cigna and Moda. Nationally, only eleven plans earned a perfect five-star rating, underscoring the rigor of NCQA’s standards.
