Why You Want a Care Manager

Managing your health can feel overwhelming at times. Between doctor visits, medications, and treatment plans, it’s easy to feel unsure about your next step. That’s where your Providence Care Manager comes in.

Our care managers are health care professionals who work with you one-on-one to make your care easier to navigate, schedule, and understand. Whether you’re working on losing weight, lowering your blood pressure, trying new medications, facing a new diagnosis, or managing a condition you’ve had for years, our team is here to walk alongside you. 

Who can benefit from care management?

You may benefit from care management if you:

• Need to see a doctor or specialist but don’t know where to start

• Have a chronic health condition like asthma, diabetes, or heart disease

• Are managing a serious illness such as cancer

• Are recovering from a hospitalization or surgery

• Need help coordinating multiple providers or specialists

• Have questions about your medications, treatment options, or lifestyle changes

• Are living with a mental health or substance use condition

• Need support coordinating resources for yourself or a family member after a life-changing event

What does a care manager do?

Our care managers can help you with anything, providing support that goes beyond your doctor’s office. They can help with:

• Scheduling: You don’t have time to call around, we’ll do it!

• Personalized education: How explore with you and identify how a health condition affects you, your lifestyle, your healthy balance.  We focus on your experience and then work on goals that fit your needs.

• One-on-one support: Phone, email, once a week, twice a month – you tell us what works for you.  We are here to help you feel confident in your care decisions.

• Care coordination between your doctors, specialists, and other providers

• Lifestyle guidance on nutrition, exercise, and healthy living to reduce risks

• Getting Care: Treatment options, medications, health insurance coverage and authorizations, and services in your community.

Transition of care support

If you’re switching health plans, finding a new provider, or continuing treatment while transitioning to Providence Health Plan, our team will help ensure your care is uninterrupted. This includes working with your providers to transfer records, coordinate coverage, and keep your treatment on track.

Common questions we hear:

• Can you help me find the right provider? Yes, our team will work with you to find a care team that fits your needs.

• I have a new diagnosis. What now? We’ll guide you through understanding your condition, treatment options, and available resources.

• My doctor says I need prior authorization. Can you help? Absolutely! We’ll walk you through the process.

• My medication is too expensive. What can I do? We’ll help you explore lower-cost options and financial assistance programs.

• I need medical equipment but can’t get answers. We’ll step in and coordinate with providers and equipment companies to get what you need.

• I’ve had a major life event and don’t know where to start. We’ll connect you to the right care and resources – medical, financial, and community-based.

No cost, always optional

Care management is available to all Providence Health Plan members at no cost. Participation is voluntary, and you can opt out at any time.
